A trimmer is a miniature adjustable electrical component. It is meant to be set correctly when installed in some device, and never seen or adjusted by the device’s user. Trimmers can be Variable Resistors (potentiometers) or variable capacitors.
The middle lead is for the adjustable capacitance.
The current has to run through the top and out the bottom, if you want the regular capacitance of the it then just use the top and bottom. But to help tune circuits, there is a screw to help change the capacitance and the middle lead is what you use. The current has to still go on the top, and the output from the bottom.
